The Add/Drop/Withdrawal Form should be used for adding/dropping course(s) for all colleges. For complete withdrawal from the University, please contact the dean’s office for your academic college.
It is the student’s responsibility to obtain the required signatures and approval on the form. Incomplete forms will not be processed. The completed form can be submitted to the Office of the University Registrar in one of the following ways:
