The Add/Drop/Withdrawal Form should be used for adding/dropping course(s) or withdrawing from the Graduate School and Colleges of Pharmacy, Nursing, Health Professions and Public Health. College of Medicine and College of Pharmacy students should contact their respective college for the withdrawal process and forms.
It is the student’s responsibility to obtain the required signatures and approval on the form. A Drop/Add Form without all the signatures and approvals is considered as incomplete and will not be processed.
The completed form can be submitted to the Office of the University Registrar by one of the following ways:
